Home Care Coordinator/Nurse – Northern Suburb

• Working 30 -38 hours pwk – Monday to Friday

The Role:
As a highly valued and compassionate Home Care Package Coordinator working with a small team, you will provide support to customers living in the North Metro area and work closely with broker partners to ensure all care and services are monitored, evaluated and achieved. You will be responsible for: assessment and coordination of customer services; developing and maintaining customer support plans; managing a case load of up to 60 customers; reviewing support plans, risks and adapting to changing requirements; working with customers, families and support staff; providing some clinical services within the scope of practice as a Nurse; reviewing and following up customers clinical issues, identified risks, or incidents; maintaining and monitoring compliance as per Aged Care and Quality standards.

​The Successful Candidates:

• Registered Nurse with current AHPRA registration
• Experience in aged care and home care setting
• Sound understanding of Aged Care Quality Standards
• National Police Clearance
• Current WA Drivers Licence
• Current influenza and COVID vaccinations notifications (or approved exemptions)
